Medical Authorization
A Medical Authorization is a written prescription from a doctor authorizing the purchase of an AED. Per the FDA, HeartSafe America must obtain a signed Medical Authorization before your AED can be shipped. Prescription expceptions:
- Philips HeartStart OnSite does not require a prescription unless infant/child pads are purchased.
Attached is a blank Medical Authorization to be completed by your doctor and faxed back to (972) 247-9417.
